Case 1: An Ex‐Smoker with Weight Loss

CASE

A 72‐year‐old man presents to the clinic with a history of 10 kg weight loss. He is an ex‐smoker. His appetite is reduced. He reports fatigue and nausea but no vomiting and has recently developed constipation. On examination he has marked proximal muscle loss and mild dependant oedema. Abdominal examination is normal.

Blood tests showed:

WCC 7.2 × 109/L
Platelets 450 × 109/L
Na 130 mmol/L
K 4.8 mmol/L
Ca 2.56 mmol/L
Urea 3.4 mmol/L
Creatinine 110 µmol/L
