Case 49: A Patient with Progressively Worsening Loss of Vision in One Eye
CASE
A 64‐year‐old smoker presents with loss of vision in his left eye which has been worsening over the past several weeks in a step‐wise fashion, affecting especially the inferior field. The right eye is not affected and he has no pain or headache. He has also noticed pain in his right leg when he walks more than 200 yards, relieved by rest. Otherwise he is well apart from long‐standing shortness of breath and productive cough, related to smoking. Examination shows a normal right eye. Left vision is 6/36 with an inferior visual field defect coming up to the horizontal through fixation. His fundus shows a pale, partially swollen left optic disc with flame haemorrhage on the superior margin.
