Case 48: A Patient with Sudden Loss of Vision in One Eye
CASE
A 64‐year‐old smoker presents with sudden painless loss of vision in his left eye. He remembers seeing a goal scored in the TV football last week, after which he lost vision completely for several seconds in the same eye. This time, now two hours since onset, the vision has not returned. The other eye is asymptomatic and he has no other symptoms. On examination he has perception of light only in the left eye with a marked left relative afferent pupillary defect. The right eye is normal. The left fundus is pale with a cherry‐red spot in the centre. He has bruits over both carotids and a short ejection systolic murmur. His BP is 150/90 mmHg.
