Case 37: A Patient with a Progressive Decline in Mobility
CASE
A 70‐year‐old woman is who is normally fit and walks several kilometres per day presents with a progressive decline in mobility over a 3‐month period such that her walking distance is limited to 100 metres. She describes that her feet feel ‘odd, as if wrapped in bandages’ and at night she is kept awake by cramps and general discomfort. On examination there is distal weakness in the lower limbs (ankle dorsiflexion MRC3/5), loss of vibration sense to the pelvis and reduced light touch to the knees. The motor conduction velocity in the lower limbs is reduced at 25 m/sec (normal 45–70 m/sec)
