Case 10: A Patient with Weakness and Wasting of the Hand
CASE
A 60‐year‐old man presents with progressive weakness and wasting of his right hand over a 6‐month period. On examination he has a diminished biceps reflex but a positive Hoffman's sign (flexion of the terminal phalanx of the thumb in response to tapping the fingers). There is marked wasting of the first dorsal interosseous muscle. Although he has no leg symptoms the tone is mildly increased and the plantars are extensor.
