Case 3: A Patient with Worsening Back Pain and Weight Loss
CASE
A 67‐year‐old man presents with worsening back pain and weight loss. He has neglected a worsening ‘smoker's cough’, and had two episodes of haemostysis. He also complains of thirst, constipation and polyuria. He is dehydrated, and appears anaemic, chronically sick and cachectic. There is thoracic spine tender to percussion without long‐tract signs, and clinical evidence of a left pleural effusion. A chest radiograph demonstrated a large left hilar mass, with associated pleural effusion and lytic bone metastases.
