Case 7: A Tired Patient with Lumps
CASE
A 29‐year‐old man originally from Tanzania presents to his GP with lumps under his arms and in his neck and groins. He has also been feeling very run down over the last few months and has lost 12 kg in weight. He works as a porter in the local hospital and he finds the work difficult and stressful. He reports no night sweats or fevers. A review of systems was unremarkable. On examination he had oral candidiasis and widespread, small‐volume lymphadenopathy which was non‐tender. Cardiovascular, respiratory and abdominal examinations were all normal. In particular, there was no organomegaly.
Blood tests showed:
Hb 10.5 g/dL MCV 94 fL WCC 3.8 × 109/L Platelets 135 × 109/L
Electrolytes, liver function tests and CRP: normal
