Case 18: A Patient with Burning Pains in Her Fingers
CASE
A 75‐year‐old woman presents to her GP with a 2‐month history of burning pains in her fingers and generalized itch. She is otherwise well although not getting much sleep due to her symptoms. She has no history of thrombosis, diabetes or hypertension. She also notes that the itchiness is worse after a bath. Cardiovascular and respiratory examinations were normal. On palapation of the abdomen, a 2‐cm spleen could be felt but there was no hepatomegaly.
Blood results came back as follows:
Hb 14.2 g/dL WCC 13.2 × 109/L Platelets 1654 × 109/L
Film: variation in platelet size, basophilia
U&E, LFT, calcium, CRP normal
