Case 2: A Young Man with a Sore Throat and Dark Urine
CASE
A 20‐year‐old man had a sore throat 1 week ago. He was treated with antibiotics in the community and his symptoms improved. He presented 10 days later to A&E with dark urine and abdominal pain. On examination he appeared relatively well at rest. He was afebrile and had a normal blood pressure. There were no murmurs heard on examination. The following results were available to you:
Hb 13 g/L WCC 12 × 109/L Platelets 240 × 109/L CRP 40 mg/L ESR 59 mm/h Na 138 mmol/L K 4.1 mmol/L Urea 8 mmol/L Creatinine 120 µmol/L LFTs Normal
Urinalysis: microscopic haematuria, protein ++
Blood culture: no growth
ASO titre: positive
