Case 15: A Traveller with Diarrhoea and Fever
CASE
A 25‐year‐old backpacker had been travelling in Malawi for a month and arrived back in the UK 3 weeks ago. He had been fully vaccinated prior to his travels and was diligent in taking his malaria tablets. He stayed in semi‐rural areas and ate local cuisine. He developed bloody diarrhoea 2 weeks after arriving back and became increasingly lethargic with occasional rigors. Despite eating lots he could not keep his weight on. There was no rash or abdominal pain. There was no history of sexual contact whilst in Malawi. He has never had diarrhoea before and there is no family history of inflammatory bowel disease.
Examination revealed a low‐grade fever but was otherwise normal. There was no rash, lymphadenopathy or organomegaly on abdominal examination. He was not jaundiced. He was HIV negative, his blood culture was negative and three thick films for malaria parasites were negative.
