Case 14: A Patient with a Fever, Sore Throat and Rash
CASE
A 35‐year‐old businessman presented to his GP with an acute onset of fever, sore throat and a rash across his trunk. He also complained of muscle ache and loose stool. He has frequented Thailand in the last 6 months to establish business links. He was last there 6 weeks ago. He admitted to having sex with a prostitute during this visit. He is married with a 1‐month‐old baby.
On examination he was febrile and there was evidence of oral Candida. He had cervical and inguinal lymphadenopathy and a maculopapular rash on his trunk. His tonsils were swollen and red. His chest was clear and abdominal examination was unremarkable. He had mild lymphopenia, but otherwise his full blood count/urea and electrolytes/liver function tests were normal. He also had a negative monospot test.
