Case 4: A Patient with Confusion, Weight Loss and Agitation
CASE
A 60‐year‐old woman is transferred as an emergency to casualty with confusion, weight loss and agitation. On examination she is cachetic and is sweating profusely. Her heart rate is 140 beats/min and she has difficulty breathing. An ECG shows atrial fibrillation, and chest X‐ray demonstrates an enlarged heart with bibasal pleural effusions. A diffuse shadow is also noticed in the upper mediastinum. Further careful examination of the patient reveals a large scar previously ‘hidden’ underneath her necklace.
