Case 7: A Man with ‘Coffee‐Ground’ Vomiting
CASE
A 78‐year‐old man is admitted to hospital with ‘coffee ground’ vomiting. This had become an increasing problem over the previous 6 months. He had been experiencing bloating after meals associated with nausea and vomiting. He had lost 6 kg in weight. On examination it is evident he has lost weight. He has no palpable lymph nodes. Abdominal examination including rectal examination is normal.
Blood tests showed:
Hb 9.2 g/dL MCV 72 fL WCC 9.8 × 109/L Platelets 180 × 109/L Na 140 mmol/L K 2.9 mmol/L Urea 12.4 mmol/L Creatinine 100 µmol/L
An abdominal X‐ray shows dilated loops of small bowel.
