Case 12: A Man with Rectal Bleeding of Increasing Frequency
CASE
A 72‐year‐old man is seen in clinic and reports rectal bleeding. This has been occurring intermittently over the previous three to four months but has become more noticeable. Now the bleeding is seen on most episodes of defaecation. The blood is bright red in colour and seen mixed in with the stools. The patient also reports urgency and a sense of incomplete emptying (tenesmus). Examination demonstrates no lymphadenopathy and no palpable abdominal mass. Rectal examination shows some altered blood on the glove.
Blood tests showed:
Hb 9.6 g/dL WCC 7.3 × 109/L Platelets 190 × 109/L Na 140 mmol/L K 4.2 mmol/L Urea 4.2 mmol/L Creatinine 96 µmol/L
Plain X‐ray of the abdomen is unremarkable.
