Case 10: A Man Who Has Vomited Blood
CASE
A 54‐year‐old man is admitted to hospital after vomiting about 500 mL of clotted blood. He has been brought to hospital by ambulance. The crew report that he was found in a collapsed state outside a local off‐licence. On examination he is obtunded and smells of alcohol. He has a metabolic flap. He is febrile and tachycardic. His pulse is 120 beats/min and his lying BP 100/60 mmHg. He appears to be jaundiced and has palmar erythema and bilateral Dupytren's contractures. Abdominal examination reveals hepatomegaly and some dullness to percussion in the flanks.
Blood tests showed:
Hb 6.4 g/dL WCC 14 × 109/L Platelets 76 × 109/L PT 36 sec Na 116 mmol/L K 3.2 mmol/L Urea 5.4 mmol/L Creatinine 90 µmol/L
