Case 21: A Child with Fever, Irritability and Vomiting
CASE
A 4‐year‐old boy presents to A&E with a 2‐week history of fever, irritability, vomiting and right upper quadrant (RUQ) pain. He had a past medical history of intermittent loose stools and failure to thrive, as well as two previous superficial skin abscesses which were treated by incision and drainage. He had also had one episode of lymphadenitis and biopsy showed the presence of granulomas. He has one sister who is well and one brother who has also had a superficial abscess. On examination he was noted to have a temperature of 38°C, was tachycardic with mild hypotension. His oxygen saturations were reduced on room air. He was found to have an enlarged liver and ultrasound demonstrated the presence of two liver abscesses.
