Case 18: A Patient with Chronic Chest Pain
CASE
A 48‐year‐old man presents to hospital with chest pain. The pains started 3 years ago, when he experienced 2 or 3 episodes, each lasting about 10‐20 minutes. They were felt retro‐sternally, described as being tight, without radiation, of a sort he had never experienced before. They were not effort or respiration related. They were quite severe, and he was admitted to hospital. He is known to have a high, though untreated, cholesterol, high blood pressure and is a smoker. Examination, chest X‐ray, full biochemical and haematological screen, and ECGs throughout the 24 hours of his admission were normal and troponin was not raised. His symptoms did not recur during hospitalization and he was discharged home.
