Case 16: A Patient with New Onset Chest Pain
CASE
A 48‐year‐old man is sent by his GP for outpatient evaluation of chest pain. He has noticed chest pains for some 6 weeks. The pain is left sided, described as being ‘like toothache’, and occurs in episodes lasting about 45‐60 mins. He has not identified any clear provoking or relieving factors; his exercise capacity has been unchanged, and he regularly walks the 2 miles to his manual job. He is a smoker of 20 cigarettes a day, he has had ‘borderline’ hypertension, not treated, for several years, and his total cholesterol is 5.5 mmol/L. There is no family history of premature coronary disease. He drinks 30 units of alcohol per week. Examination shows he is 1.80 m tall, BMI 35, heart rate 70 beats/min, BP 145/90 mmHg, the rest of the exam is normal.
