Case 14: A Patient with Calf Pain and Loss of Appetite
CASE
A 76‐year‐old woman was referred to the medical take by her GP with a 2‐day history of lower left calf pain and possibly some swelling. Systems questioning reveals that she has been losing weight over the past few months but she attributes this to feeling less hungry and has had some lower abdominal pains due to constipation. She lives independently but states that she lacks energy of late. She has hypertension which is well controlled with bendrofluazide. A diagnosis of osteoarthritis has also been made which affects her hand and knee joints in particular.
Examination reveals that she is cachectic, and there is tenderness in the mid and upper left calf. The left calf is swollen in comparison to the right and there is some increased warmth. Varicosities of both legs are noted. Examination of her cardiovascular and respiratory systems are normal. She has some tenderness on palpation of the left iliac fossa but no masses.
