Case 11: A Drug User with a Cough and Chest Pain
CASE
A 26‐year‐old known intravenous drug abuser presents with a long history of cough, pleuritic chest pain and occasional haemoptysis. He also describes night sweats and fevers.
Examination reveals a cachectic man who is pyrexial (temperature of 39.2°C). He has a raised jugular venous pressure and pansystolic murmur, which is loudest over the left sternal edge and accentuated by inspiration. His C‐reactive protein (CRP) is 160 mg/L, and his white cell count (WCC) is 18.4 × 109/L. Chest X‐ray is normal.
